Additional Requirements for King-May Products (Applicable to King-May products only)

If you are ordering King-May products, please provide the following supporting documents based on your situation:

A. First-time Purchase

• Mandatory Attachment: A copy of your User Certificate.
• If the certificate is unavailable: Please provide your [Registration No.] and [Registered Email with King-May] in the section above.
• If both are unavailable: Please contact King-May directly to retrieve your records before we can proceed with your order.

B. When Purchasing a “Kit”

• Individual Purchase: No purchase agreement form is required.
• Organization Purchase: An **[Institutional Purchase Agreement]** must be submitted.

**Note on Submission: The original physical copy of this form must be mailed to our office: Room 601, 6/F, Pan Asia Centre, 137 Wai Yip Street, Kwun Tong. To expedite the process, please email a scanned copy of the completed form to info@edgecs.org for pre-review before mailing the original.

**Note on How to Obtain: This form is not available for public download. The form will be sent to you via email once we receive an organizational email order for any kit.

**註：如何索取： 此表格不設網上公開下載。當我們收到機構發出訂購任何 Kit 套裝的電郵後，我們將會把表格電郵發送給您。
